document v3.1.1 fixes in CONTINUITY — finding 1 (home packages descend into home-manager-path env + dedup) and finding 2 (doctor config-syntax: root cause 'nixos-config not in NIX_PATH', nina fix = flake-aware dry-build, golden fix = restore nixos-config to nix.nixPath in configuration-unstable.nix; golden not yet re-baked so channel-style doctor on /etc/nixos still fails until rebuild). Both nina fixes verified real on the unstable golden
