docs: add Codex Security scan report (2026-08-03)

Read-only standard scan of the whole repo (scan b65982d1) against koh head 0ae03db7 / v2.10.15: 4 reportable findings (1 medium, 3 low, all high confidence) plus 4 suppressed with policy rationale. Report landed at docs/peach-android-security-scan-2026-08-03.md; sealed artifacts and SARIF remain in the scan temp dir. Also ignore *.swp editor swap files and record the outcome in CONTINUITY.md. No code changes; no version bump (docs-only).
