fix login/setup/TOTP pages ignoring the operator theme: appendAuthHead now injects the derived <style id=kt> theme override (app.theme_css) before </head>, the same block writePublicHtml adds to public pages — so the auth pages track the chosen --accent instead of rendering style.css's default amber ramp (v1.56.181)
