add remembered secret-scan false positives (schema v35 secret_false_positives) — marking a finding a false positive in ops now persists by (repo,file); every re-scan (incl. the 6h trust scheduler) re-files it as acknowledged so it never re-dings the signal; restore clears the mark; bump v1.39.18
