globally ignore .kohignore + .agent/ in kepr: manifest.parse compacts them out of the file list (covers atlas, file viewer, source archive, safety scan, line count for ALL repos immediately — no re-offer needed) and dropIgnored runs on the ingest paths (handleComplete + contributor promote) so they never enter save rows/object refs. add isGloballyIgnored + regression test. also includes finding-3 ack backend groundwork (db v36 code_finding_false_positives table + trust derivation skips acked findings; ops UI still pending). bump v1.39.30
