add a full ops issue management system: cross-repo triage inbox (/ops/issues) with status segmented control + live counts, repo/type/severity filters, search, bulk status/delete; per-issue thread+triage view (/ops/issues/:repo/:n) with emailed replies (reuses operator-aware public reply route), status/severity/type triage, operator-only internal notes (stored in the separate issue_notes table so they never leak publicly), attachments, delete. backend: db listAllIssues/issueStatusCounts/distinctIssueRepos/setIssueSeverityType/internal-note fns + ops endpoints opsAllIssues/opsIssueDetail/issueSetMeta/note CRUD/opsIssuesBulk; per-repo 'manage issues' link in RepoDetail; new SolidJS Issues + IssueThread pages, nav entry, shared issueMeta. bump v1.39.29
