koh offer --all: clean header + single self-overwriting progress line (TTY) instead of a line per save; suppress per-save HTTP errors in batch (quiet flag through offerSaveToRemote/twoPhaseOffer/failOffer) and roll them into the summary with a pointer to a single offer; also fixed the bare-host repo name in the 4th offer path (v1.18.4)
